To Schedule an Appointment:
Call us to find an available time that is convenient for you. Be ready to discuss with us the type of appointment you would like (for example: routine exam & cleaning, restorative, emergency, etc.). If you are calling off-hours, please be sure to leave us a message regarding the nature of your call and we will contact you when we return to the office.
Please note: new patients having an emergency will only be seen during regular office hours.
What you should know.....
Some things we would like you to know prior to your first visit are:
1) We adhere to our schedule and rarely get behind. If we do, we will let you know. In return we ask that you contact us if you will not be able to make an appointment....someone else may need the time.
2) If you are scheduling for a first time exam & cleaning appointment, that's what you can expect us to do at this appointment....clean your teeth!
3) NO SILVER FILLINGS! All restorative (fillings) are completed using composite materials for a more conservative and better restoration. We do not use amalgam. Please see our "Why Composites?" document for a detailed explanation outlining the benefits.
To Cancel a Scheduled Appointment:
We believe YOUR time is as valuable as OUR time. If you find that you are unable to keep an appointment, please call us as soon as possible -- preferably 48 hours in advance of your appointment. Routine cancellations or "no shows" will be charged a fee after 3 missed appointments.
Some appointments require that a patient pre-medicate with antibiotics due to certain health conditions. Ultimately, it is your responsibility to know if you need to or not -- please contact your treating physician's office to verify.
Congratulations! Definitely let us know the trimester you are in as we may need to see you at another time. If you are having a dental emergency, we will also need to know who your ObGyn is, their phone number, and may need to obtain clearance with your ObGyn to treat you medically.
Please bring your insurance card to each appointment. If you received a new card from your insurer, we will need to see it and make a copy for our files.
It is your responsibility to know the basics of your plan; such as your carrier, your maximum benefit amount and plan renewal date, and whether or not you have a deductible. We deal with many insurance plans that vary from subscriber to subscriber, even within the same insurance company.
All insurance companies request "pre-determinations" to be submitted for certain procedures and those $300 and over. Please note: this is not a guarantee of payment. If you enter treatment and the insurance company does not pay, you are responsible for the unpaid portion.
All dental insurance companies now have the ability for members to create user accounts and keep track of their benefit usage. We urge you to sign up and be involved, especially if you will be using your benefit for major work and visiting several dental specialists.
All fees or insurance deductibles are due at the time of service for most treatment plans. If you have insurance, we will submit your claim and have the insurance company pay us directly, then we will bill you for any coinsurance or amount remaining unpaid. All statements are due within 15 days and will include a finance charge for any outstanding balance.
We accept cash, check, debit, VISA, MasterCard, Discover & CareCredit.
Should you need a payment plan, we offer CareCredit. They have several "No-Interest" and "Extended Payment" plans that can easily fit any budget. To apply, visit www.carecredit.com or call them at 1-800-677-0718.
